📲 Portal Access & Appointment Steps

At Shine Thru Health NP, we’ve made it simple to get started with virtual care. Follow these easy steps to access your secure patient portal and prepare for your appointment with Marquita.

4-Step Process

Step 1: 📅 Schedule Your Appointment – Book via Calendly or as instructed by our office.
Step 2: ✉️ Activate Your Portal – Accept your secure CharmHealth invitation and set up your account.
Step 3: 📝 Complete Forms – Sign consent forms and fill out intake questionnaires before your visit.
Step 4: 💻 Join Your Telehealth Visit – Log in and click the secure Telehealth link at your appointment time.

✅ Step 1: Schedule Your Appointment

Choose the time that works best for you:

  • 📅 Book via Calendly (insert your booking link)

  • Or schedule directly through CharmHealth if instructed by our office.

✉️ Step 2: Set Up Your Patient Portal

The CharmHealth Patient Portal (PHR) can only be created through a secure email invitation from our practice.

How to register:

  1. Watch for your invite email after booking.

    • Check your spam/junk folder if you don’t see it.

    • If you haven’t received one, contact us so we can resend it.

  2. Click the secure invite link in the email.

  3. Enter your date of birth in mm-dd-yyyy format, then click Next.

  4. Create your password, following the password rules shown.

  5. Complete the CAPTCHA.

  6. Accept Charm PHR’s privacy policy, terms, and conditions.

  7. Click Submit.

  8. Enter the verification code (OTP) sent to your email.

  9. Click Verify OTP to finish.

Once complete, you’ll receive a Welcome Email from Charm with your personal PHR sign-in link for future use.

📝 Step 3: Complete Forms Before Your Visit

Before your appointment, log in to your portal to:

  • Sign required consent forms

  • Complete any intake questionnaires

  • Update your medical history if needed

 

This ensures Marquita has everything ready for a focused, personalized visit.

💻 Step 4: Join Your Telehealth Visit

On your appointment day:

  1. Log in to your CharmHealth portal.

  2. Click the secure Telehealth link.

  3. Connect with Marquita for your private virtual visit.
